Advance care planning ensures that others – loved ones, family
members or close friends – are prepared to act on behalf of someone
when they can’t speak for themselves. It’s a difficult conversation to
have, and it does take courage, which is why it’s often called “The
Courageous Conversation.”
Our goal is to normalize these discussions with ALL Adults (anyone 18 and older)
The Lake Taylor Transitional Care Hospital Foundation is presenting the
Kay and Al Abiouness Advance Care Planning Seminar. This seminar
will help guide and prepare individuals and families to have a
“courageous conversation” with their loved ones to learn about their
wishes and help them make an Advance Care Plan.
